Registering with us

When you attend the Practice to register you will be asked to complete a GP registration form, and new patient questionnaire form, these must be completed before you see the Doctor, so it is advisable to attend at least 10 minutes before your appointment time, so that you can compete the information required.

The receptionist will offer you a new patient appointment. Acceptance on to the list is as at the discretion of the Doctor. We will ask you for proof of identity.

There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case simply choose another GP in your local area.

Practice Area

We look after patients in the following postcodes: G1, G2, G3, G4, G11, G12, G20, G21, G22, G23, G64.

Your medical records may take several weeks to arrive at the Practice.

NHS Medical Card

There is now no requirement for a patient medical card, and these are no longer issued to patients by Greater Glasgow and Clyde Practitioner Services Division. Once you have completed the required registration forms and have been accepted on to our list you are then registered with our Practice.

Please note we do not issue any type of form/card to indicate who your registered doctor is.
